The researchers hypothesize that without their usual social support systems in place, due to lockdowns, people suffering from those disorders found it much more difficult to manage their conditions. In those groups, the researchers found posts about hyperfocusing on the news and relapsing back into anorexia-type behaviors since meals were not being monitored by others due to quarantine. The researchers analyzed posts from 15 subreddit groups devoted to a variety of mental illnesses, including schizophrenia, depression, and bipolar disorder. They also included a handful of groups devoted to topics not specifically related to mental health, such as personal finance, fitness, and parenting. Virtual computer analysis suggests that if you took the regions of the brain in that network out of the equation, the other brain regions would not be able to pick up the slack in mind-wandering.
Practical Considerations for Caffeine Use in ADHD Management
The authors also analyzed psychometric curve parameters, common in psychophysics, but not yet applied to ADHD. After the neurofeedback training session ended, the researchers asked subjects to perform two additional tasks that involve attention, and found that the enhanced attention persisted. In one experiment, subjects were asked to watch for a grating pattern, similar to what they had seen during the neurofeedback task, to appear. In some of the trials, they were told in advance to pay attention to one side of the visual field, but in others, they were not given any direction. Previous studies have shown a strong correlation between attention and alpha brain waves, particularly in the parietal cortex. In humans and in animal studies, a decrease in alpha waves has been linked to enhanced attention.
Ultimately, the dependence on caffeine negatively impacts the sleep-wake cycle in the brain and increases alertness in the short term. But this benefit eventually leads to drowsiness, anxiety and impulsivity, which isn’t all that great for a person’s attention span, ADHD or not. Two other studies used hot beverages for dose administration 59,60, and one study 56 used 100 mL of a solution to dilute the supplements. Regarding caffeine’s side effects, in two adhd meds and coffee of the studies included in our review 56,59, none of the participants reported side effects. In another study 54, several children mentioned experiencing slight discomfort. Arnold et al. 52 observed weight loss in some individuals, while the other three studies 57,58,60 did not mention any side effects. Attention deficit hyperactivity disorder (ADHD) is marked by difficulty maintaining focus on tasks, and increased activity and impulsivity.
Pharmacologically, methylphenidate acts on the dopamine and norepinephrine transporters, inhibiting the reuptake of these substances in the terminal neuron of the synaptic cleft and thus increasing their availability for synapses 20. With more dopamine and norepinephrine present in the synapse, the transmission of these neurotransmitters is facilitated, improving communication between neurons 21. This substance, however, can also cause decreased appetite, insomnia or sleep disturbance, nausea, tachycardia, increased blood pressure, mood swings, weight loss (at higher doses), and even possible growth reduction 22. Recent studies have shown that medication for ADHD treatment can reduce the risk of accidents and unintentional injuries in children, adolescents, and adults with ADHD 14.

In contrast, a comparison of 12 children treated with caffeinated vs. decaffeinated coffee significantly improved general ADHD symptoms 75. The controversial effect of caffeine on patients with ADHD is also seen in the literature for the adult population. A cross-sectional study involving 2259 adult patients with ADHD concluded that caffeine does not seem to be successful for self-medication of ADHD symptoms 76.

Enhanced alertness and energy levels are another frequently cited positive effect of caffeine on ADHD symptoms. The stimulant properties of caffeine can help combat the fatigue and low energy levels that often accompany ADHD. This boost in energy can be especially helpful for individuals who experience the inattentive type of ADHD, characterized by difficulties with focus and organization rather than hyperactivity. The risk of bias evaluation was conducted independently by two authors (G.P. and M.M.G.M.) using the revised Cochrane Risk of Bias 2 tool (RoB-2) 55 for the quality assessment of the randomized studies. The Cochrane RoB-2 (Risk of Bias 2) tool was designed to provide a standardized and transparent framework for assessing the quality and reliability of scientific studies 55.

Furthermore, exclusion of artificial food colors and supplementation of free fatty acids, particularly omega-3 and omega-6, showed promise in reducing ADHD symptoms 83. Pharmacological treatment is the first choice for children above 5 years old with ADHD 16,17,18,19. First-line drugs include psychostimulants, while non-stimulants (such as guanfacine, atomoxetine, and clonidine) are considered second-line options for pharmacological treatment of ADHD in children above 5 years old 18.
This difference in mechanism of action means that caffeine is generally less potent and specific in its effects on ADHD symptoms compared to prescription medications. While energy drinks contain caffeine and may provide a temporary boost in focus and energy, they often also contain high levels of sugar and other stimulants. These additional ingredients can lead to energy crashes and potential negative health effects, making them a less ideal choice for long-term ADHD management compared to other caffeine sources. While many individuals report positive effects, the efficacy of caffeine can vary significantly from person to person.

However, this link was not consistently observed across all cognitive domains that were tested 68. Five of the analyzed studies 52,54,57,59,60 included a general symptom range. Three 54,57,59 of them did not identify any difference in general ADHD symptoms, regardless of the scale applied. Arnold et al. 52 applied five checklists to evaluate general symptoms after the period of caffeine administration in patients, with a significant improvement result only in the “Parents symptom checklist reported”.
